Return Authorization Request

Obtain authorization before sending an instrument to JDSU for service.

Once you submit this form, please do not ship the instrument to us until you receive a return authorization (RA) number. You can use this number to track your instrument while it is in for service, calibration, or upgrade. If you do not receive a follow-up call or if this is an urgent issue, please contact Customer Service at 1-866-228-3762 option #2. JDSU offers two day expedited service in the U.S. for may instruments, available at an additional charge. If you would like to receive expedited service, please indicate your choice below. Please submit one form per instrument.

  How to find your serial number
Calibration Service: Would you like for your instrument to be calibrated while it is at JDSU? If yes, you will be contacted with the cost of the calibration and a purchase order will be requested at that time.

Expedited Service: Would you like to receive two-day expedited service? If yes, you will receive a phone call that your unit can be expedited and what the charge will be.

In-Warranty Units: you will receive an e-mail message back with an RA# and the shipping address of our Product Service Center.

Equipment failure due to user negligence and unauthorized alterations or repairs (to instruments and options) automatically voids the remaining warranty. Units that are operated or installed in ways other than outlined by JDSU's literature or operating manuals also are not covered.

Out-of-Warranty Units: you will receive a phone call within one business day with the repair cost, and if you choose to move forward an RA# will be issued. The shipping address of our Product Service Center will be provided. A purchase order will be requested at that time.
