With the Online RMA Status Lookup you can instantly check the status of your repair/calibration.
Obtain authorization before sending an instrument to JDSU for service.
If you are experiencing technical difficulties with JDSU equipment, please call our TAC department at 1-800-543-1550 and select option 3 for assistance in troubleshooting your unit prior to RMA issuance. Doing so will help to pinpoint the exact cause of your trouble and assist in expediting service.
Once you submit this form, please do not ship the instrument to us until you receive a return authorization (RA) number. You can use this number to track your instrument while it is in for service, calibration, or upgrade. If you do not receive a follow-up call or if this is an urgent issue, please contact Customer Service at 1-866-228-3762 option #2. JDSU offers two day expedited service in the U.S. for may instruments, available at an additional charge. If you would like to receive expedited service, please indicate your choice below. Please submit one form per instrument.
