Below is an Return Material Authorization request form that will be transmitted to your local Customer Care department. Once this form is received and your information is validated, Customer Care will respond with an RMA number. You can use this number to track your instrument while it is in for service, calibration, or upgrade. If you do not receive a follow-up call or email or if this is an urgent request, please contact Service Center (LOCAL REGIONAL PHONE NUMBERS). JDSU offers two day expedited service in the U.S. for many instruments which is available for an additional charge. If you would like to receive expedited service, please indicate your choice below.
