How to Return Merchandise for Repair and Calibration
Please complete the following form to obtain authorization prior to shipping your instrument to JDSU for repair or calibration. Do not ship the equipment until you receive the Return Authorization (RA) number from JDSU. You may use this number to track your equipment during repair, calibration, or upgrade. If your request is urgent, please contact Customer Care.
We offer two-day expedited service in the United States for some instruments at an additional charge.
